Neurological Stroke, Coma, Dementia, Parkinson's

  • FAST scale > 7
  • Inability to speak < 6 words
  • Inability to walk without personal assistance
  • Loss of ability to sit up without assistance
  • Loss of ability to smile
  • Loss of ability to hold head up independently
  • Difficulty swallowing
  • Refusal to eat
  • Incontinent of bowel & bladder

Lung

  • DOE and at rest
  • Decreased breath sounds
  • Increased cough/sputum
  • Cyanotic extremities
  • Cor pulmonale/ RHF
  • Oxygen dependant
  • PCO2 > 50mm Hg
  • FEV1 < 30%
  • PO2 < 55mm Hg on oxygen
  • O2 Sat < 88% on oxygen
  • Resting Heart Rate > 100bpm
  • Steroid dependant
  • ECHO; EKG; CXR

Debility Unspecified

May be used if primary diagnosis is not considered to be terminal. The patient may have several co-morbidities that together may justify a terminal prognosis.

Diabetes

  • Unstable blood sugars
  • Vascular insufficiencies; wounds
  • Gangrenous wounds
